Insurance and Payment Information at PCA

To help PCA better serve you, please provide your insurance card and benefit plan information to our staff. If applicable, this should include co-pays, co-insurance percentage, deductible amounts and if they have been met yet, and any services that are not covered under your plan.
We require that all patients pay their balances promptly. If your insurance has a deductible that has not been met, you may be asked to pay for any services when a deductible will be applied. All co-pays are to be paid at the time of service.
If any of our patients have a balance due, you may be asked to make a payment on your account prior to scheduling your appointment or at time of service. This payment is in addition to any co-pays or payments for services provided at your scheduled appointment.

Payment Options

Payment is due at the time of service. Patients that have HSAs, HRAs, or FSAs can provide this information to our staff and we will enter securely on your account for any balances that may become due. Credit Cards and Debit Cards can also be kept on your account with set Payment Plans. Please speak to our receptionist or the billing department if you are interested in this.

Thank you for allowing PCA to be your Health Care Provider and we appreciate your patronage. Our physicians contract with most of the health plans in the area with a few exceptions. If one of our doctors is not listed as a provider for your health insurance plan, please call us to inquire if we are able to accept your insurance. 

As a rule, your insurance pays for many of your medical services delivered in our office. However, depending on your health insurance plan, there may be deductibles, co-insurance and co-pays that are your responsibility. Please be aware that most insurance plans require that your office visit co-pay amount be paid on the date of service. We will do our best to make you aware of the amount due as required by your insurance.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. If you don’t have insurance or don’t intend to use insurance to pay for scheduled non-emergency healthcare services, federal law requires that healthcare providers and facilities provide you with an estimate of the expected charges for medical items and services at least 1 business day before the scheduled services are to be performed. 

  • If you are uninsured or not using insurance to pay for your health care services and receive a bill that is more than the Good Faith Estimate, you have the right to dispute the bill. 
  • Any patient may request an estimate of the expected charges for non-emergency health care services that have been ordered, scheduled, or referred and state law requires that health care providers and facilities provide you with an estimate of the expected bill for medical items and services within 5 business days of the request.
  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like office visits, procedures, laboratory testing, x-ray, or diagnostic testing. 
  • Make sure to save a copy or picture of your Good Faith Estimate.

As you know, insurance is an extremely complicated system and unfortunately, errors do occur. If you feel there has been a mistake with your bill, please don't hesitate to bring it to the attention of our Billling Department.
