What will I need when checking in for my appointment?

Please allow ample time to complete the check-in process before your appointment time. 

Be prepared to show your insurance information and photo ID at the time of your visit as well as proof of residence.

There may also be additional paperwork needed.

Also, remember that your co-pay amount is due at the time of service.
